Medical homes provide personalized health care

At Madigan, patients work with their primary care teams in Patient-Centered Medical Homes as partners in care, to foster a spirit of health, wellness and trust.

This approach to health care puts patients in the driver’s seat as they partner with their providers to develop individual comprehensive care plans. These “health care blueprints” are focused on healthy behaviors, preventive health measures and chronic disease management strategies.

As a true system for health, Madigan’s medical homes are not disease centered or provider centered, but patient centered. They promote healthy changes at an individual level and emphasize lifelong health, rather than merely treatment of illnesses and injuries.

The Madigan staff has received many accolades from patients who appreciate being seen by care teams who really know and understand their wellness goals and treat them as active members of the team, not passive recipients of care.

Patients also enjoy and appreciate the continuity they receive within the PCMH teams. Each patient is assigned to a primary care team led by a physician, physician assistant or nurse practitioner. The primary care provider is supported by registered nurses, licensed practical nurses, nursing assistants, medical assistants and medical support assistants. If a primary care provider is not available, another provider on the team who knows the patient can step in and provide care.

For patients who need treatment outside of their PCMH, such as specialty or inpatient care, their care teams provide coordination and integrate services to ensure the highest-level of comprehensive care.

Patients and Madigan health care teams also take advantage of information technology to maximize patient education, documentation of care and communication. Most notably in this area is RelayHealth, which allows for secure online communication between primary care teams and patients. Using RelayHealth, patients can access health information, avoid unnecessary trips to the clinic for administrative issues, and request appointments and medication renewals.
